BIA Academic Probations: Probation Extension Appeal Request Form

This form is for students that failed to clear an ACADEMIC PROBATION status at the end of the required probation period. The student currently qualifies for termination, according to the Satisfactory Progress Policy, and will be terminated from BIA within 10 days of the date of determination. However, the student has the opportunity to apply for an appeal that would extend their original Probation period. This request must be received within 10 days from the date of determination. In order to be approved, this form must be reviewed and signed by an Academic Dept. staff member.
