    I am aware that the participation in dance rehearsals and performances for Polonia Polish Folk Dance Ensemble (Polonia) is voluntary, and involves inherent risk during participation, including the risk of possible accidents, physical injury,  as a result of attending and participating in dance. I freely accept and fully assume all such risks, dangers, and hazards, including but not limited to personal injury, death, property damage, or loss, resulting from the participation of the Participant. I have carefully considered the risks involved, and I have full confidence that reasonable precautions will be taken to ensure the safety and well-being of the Participant. 

    In consideration of Polonia permitting my participation in rehearsals and performances, I hereby voluntarily agree as follows:

    1. To assume and accept, as represented above, all risks, dangers, and hazards in connection with my participation in Polonia rehearsals and performances.

    2. To waive any and all claims- legal, equitable, or otherwise, that I may have against Polonia, its directors, officers, staff, employees, agents, representatives, and insurers (hereinafter collectively referred to as “Polonia”);

    3. To release Polonia from any and all liability for any loss, damage, injury, or expense that I or my next of kin, successors, or dependents may suffer or incur as a result of my participation in the Polonia rehearsals and performances due to any cause whatsoever.

    4. That this release and waiver agreement shall be effective and binding upon me, my heirs, next of kin, executors, administrators, and assigns, in the event of my death;

    5. To obey warning signs, if any, and other notices/instructions/regulations posted within the rehearsal facilities, or printed and/or emailed to participants.

    6. Images and videos can be taken/recorded during practices and events where Polonia participates. I give my permission to use any images and/or movies that can show me on the Polonia website (http://www.Polonia.ca), Social Media, and materials related to the promotion of Polonia.

    7. I give my permission to use my email address to send information related to Polonia.

    I have read and understood this Release and Waiver Agreement and the conditions of its operation and use prior to signing it and am aware that by signing this document, I am affecting the legal rights and liabilities of myself, my heirs, next of kin, executors, administrators and assigns in relation to Polonia.

  • COSTUME CARE AND RESPONSIBILITIES

    Each dancer is responsible for all costume pieces in their possession before/during/after performances. Costumes must be treated with respect- they are folk artifacts, authentic handcrafted pieces that represent the Polish heritage and folk culture. Each dancer (and parent of those under 18) is responsible for making sure they have all necessary pieces of costuming before each performance and is expected to return his/her costumes at the specified times outlined by the instructors.

    General Costume Care Instructions and Responsibilities

    1. All dancers must wash and iron their own white shirts and underskirts after each performance, before returning them. Dirty shirts and underskirts will not be accepted, and the dancer will then have to take it back home to wash, and then personally assure that the costume piece gets to the correct person, on time, before a performance.

    2. Cold wash and hang-dry costumes that can be washed. Do not put costumes in the dryer- they may fade and shrink. Some costumes have to be dry-cleaned, and it will be arranged by Polonia. Please ask the Director for directions.

    3. All dancers must hang up their costumes when they are being stored at home between performances and costume returns. They should not be stored in a closed costume bag.

    4. Hats should never be put inside costume bags- they break and bend.

    5. Wianki (headpieces for girls) should be stored in a Tupperware box, or be carried separately, outside of the costume bag. They should never be put inside costume bags if they are not in a hard-cover box- they break and bend, and flowers fall off.

    6. All dancers are responsible for re-sewing loose buttons and hooks before performances. ANY OTHER ALTERATIONS OR MENDING MUST BE REPORTED TO THE INSTRUCTOR AND/OR COSTUME PERSONAL.

    Do NOT alter, re-sew, hem, or do any other mending on your own, aside from re-sewing loose buttons or hooks.

    7. All dancers are responsible for ironing their shirts and underskirts, and any other costume pieces that can be ironed, before each performance. Carry shirts separately to a performance so they do not get wrinkled. Please check with an instructor or the Costume personal before ironing costume pieces that are not cotton.

    8. All girls must re-braid the hair when it is being stored. DO NOT store the hair unbraided- it gets tangled and ruined this way. Store it braided, in a Ziploc bag.

    9. All dancers must wear white or skin-colored undergarments to each performance. Colored undergarments show and look unprofessional.

    10. All dancers must clean and polish their boots before each performance.

    11. Each girl is responsible for having enough bobby-pins, elastics, makeup, and hair products at each performance.

    12. Lost or damaged costumes will be the responsibility of each dancer, and they will have to pay for having them replaced.
