Agreeing to these statements show that you understand and will follow the Utah Military Academy Blended/Online learning expectations. If understood and agreed to all statements, parent and cadet, please initial next to each statement and sign the area at the bottom of the contract.

I understand it is strongly suggested I stream my classes during their scheduled time, however if I am unable to do so, it is my responsibility to watch the uploaded video and reach out to my teacher if I have questions or am not understanding the content.
I understand that I must have access to the internet and a working computer (with a working camera and microphone) outside of the UMA Campus in order to take Blended Learning courses.
I understand that my computer camera must be turned on during class and I must be prepared, present, and in front of my computer the entire time. If I do not, I may be marked ‘absent’ for that day.
I understand that Blended Learning follows the same calendar as the on-campus curriculum and grades will be finalized at the same time as the on-campus grades. If I do not progress through my courses at the predetermined rate on Canvas, I may receive an “F” on my quarter grade.
I understand that I must take the NWEA test three times per year. I cannot opt out of the NWEA test.
I understand that I will be required to abide by all safety & wellness and uniform protocols while on campus. I also understand that I cannot be on campus at any time during the day that I am not signed up for an on campus course.
I understand that in addition to what is listed in this form, I will also be adhering to the policies and procedures laid out in the Online and Blended Learning Guide.
