    Please answer all questions carefully so we can arrange the most suitable class.

    Please note:

    • Our trial program is limited to students who meet all eligibility criteria.
    • All fees are prepaid. Classes will only be conducted after the fee is paid in full before the set due date.
    • If payment is not made by the due date, the remaining classes will be cancelled.
    • Parents who wish to pay in advance for multiple months may inform the coordinator to arrange payment.

    Completing this form does not automatically reserve a trial slot; it helps us tailor the session to your child and confirm eligibility.
