Undertaking and Acknowledgement by Student

I confirm that I will not attend the BJE and Friendship Circle events if any of the following situations arise:
- on the day of the event I have cold-like symptoms or other possible symptoms of COVID-19; and/or
- on the day of the events there are reasonable grounds to suspect that I may have COVID-19 (including that I have had a COVID-19 test for which I have not yet received the result).
I also confirm that if I am outside New South Wales within the 14 day period before any of the sessions I am scheduled to attend, that I will contact BJE before the relevant session with details of my travel so that BJE may assess whether or not it is appropriate for me to attend in view of the COVID situation at the time.

I also acknowledge that all participants in the BJE and Friendship Circle events are required to behave appropriately, and that I may be excluded from participating and/or asked to leave if I behave inappropriately on the day.

Consent to Attend, Media Release, Behaviour Requirements, and COVID-19 Undertakings

I am a parent/legal guardian of {studentsName}.

I consent to {studentsName} attending and taking part in the BJE and Friendship Cricle events on 25 October; 8 November; 22 November; and 6 December 2020.

I consent to {studentsName} being photographed and/or videod whilst taking part in the BJE and Friendship Cricle events on 25 October, 8 November, 22 November, and 6 December, and I further consent to any such photos or videos taken being used by BJE (NSW Board of Jewish Education) and/or Sydney Friendship Circle for promotional purposes. Such use may include publication in print or digital format, including online and in social media.

I acknowledge that attendance at and participation in the event by {studentsName} is conditional upon appropriate behaviour, and that inappropriate behaviour by {studentsName} may result in their being instructed to immeditely leave or stop participating in the event. Examples of inappropriate behaviour include (but are not limited to) abuse of other participants or the organisers (including their staff), wilful disobeying of reasonable instructions, and reckless or dangerous behaviour which constitutes a risk to the Student, other participants at the event, and/or staff/representatives of the organisers. Should the Student be asked to leave or stop participating due to inappropriate behaviour, their Emergency Contact will be immediately informed by phone or, if the Emergency Contact is not available to speak by phone, by text message to their mobile phone.

I acknowledge the necessity for COVID-19 precautions and confirm that {studentsName} will not attend the BJE and Friendship Cricle events if on the day they are scheduled to attend there are reasonable grounds to suspect they may be infected with COVID-19 (including, but not limited to, having been tested for COVID-19 but not yet received the test results).
