Early Years Library request form 2024/25

DBIS EYFS Library (FS1 and FS2) Book Request Form
Thank you for supporting our students' reading for pleasure and learning needs. Please use the library's catalogue https://library.dbis.edu.hk/ to identify books you would like to borrow and complete the form accordingly.
PLEASE READ CAREFULLY
1. This service is available to all DBIS families with children in Nursery and Reception.
2. Only books located at the EYFS campus library will be available for lending using this request form.
3. Please use the library catalogue to check the availabilty and location details of your requests.
4. Please only select resources with call number prefix 'K', or 'KG' listed as the location. All other books are housed on another campus.
5. Book loans are for 1 week.
6. If any book is required by a teacher for the purposes of delivering the curriculum, the Library reserves the right to request the book's return before the due date.
7. Loans may be renewed. Please complete this form with the titles of the books to be renewed and state 'renewal'.
8. Requests require 1 working day's notice.
9. We apologise in advance if any requested books are not available.
10. As per DBIS policy, invoices will be issued for any library books lost or damaged.

TYPE or CUT AND PASTE title, author and call number details below (6 maximum per family). Apologies but library staff are currently unable to choose books for families. Please provide specific titles.*
