This may not always align with the choice you've requested on this form.
Please advise us if the product being claimed was not originally sold by your store/company.
If your customer purchased the item from an overseas retailer or website such as amazon.com (parrallel importer) or from a non-authorised Spelean dealer (grey importer), please direct the customer to the retailer where they purchased the product.
Please print a copy of this form to be included with your goods being returned, or if you would like a copy of this form for the end user or your store. Please print before pushing submit.
