By selecting 'I agree', I agree to;
- The use of a face mask during transport arranged by the club
- Social distancing of 2m during activities unless further precautions are taken (use of face mask)
- Hand sanitation before and after use of club kit
- Adhere to social bubble which you travelled with and maintain this throughout the session
- Not attend if I, my household or anyone I have been in close social contact with have any Covid-19 symptoms, including a high temperature, new persistent cough or a loss in taste or smell
- Not attend if government guidance suggests you should be in a period of self-isolation
- I participate in the session knowing that this may increase the risk of infection of Covid-19 despite the precautions taken in line with the government guidance
- For my details to be used for track and trace if it is reported that a participant at the given session has tested positive for Covid-19 within a given period of time as outlined as outlined by government guidance
- To follow rules of any external facilities used during club activities
