Use this form to submit error correction requests for CLASS Portal observations that have been entered incorrectly. The LDOE Early Childhood team will contact you via the email you provide below once the correction has been made in the CLASS Portal. Error corrections should be submitted within 30 days of the error.

Types of Error Correction Requests

Error Type	Explanation	Example(s)
Observation Details Correction	An incorrect detail of the observation was entered when the observation was input in the Portal.	Observer accidentally entered their name in place of teacher name. Observer entered incorrect date of observation.
Observation Score Correction	Observer entered incorrect score in any section of the observation entry in the Portal. Specify dimension and cycle in request.	Observer entered a 7 for negative climate instead of a 1 across cycles. Observer entered a 4 and meant to enter a 5 in Quality of Feedback for cycle 2.
Observation Score Deleted	An observation was entered and should not have been. The observation & associated score needs to be deleted.	Observer entered an observation for the wrong classroom at a site. The observation needs to be deleted so that the observer can enter the observation for the correct classroom.
