Rules and Policies Agreement
I understand JobHook is a temporary/direct placement staffing company and does not guarantee steady full time work. I also understand that temporary assignments are for no specific period, and can be terminated at any time without notice or cause.
Please call us to report any of the following immediately!
- Dangerous working conditions
- If you are sick or injured on the job
- If you are going to be late or if you are unable to report to your assignment, a call must be made to our office and the job site 30 minutes before the shift to begins.
- If you feel that you cannot complete an assignment, you need to call before leaving the job site.
Company policies
Insubordination will not be tolerated toward JobHook employees or clients while on assignments. Unwillingness to take orders that pertain to your assignment can result in termination from assignment and job hook.
No possession of illegal weapons on site
No theft from client property.
At the completion of each assignment, I understand that it is my responsibility to notify JobHook my availability for reassignment.
I understand that I may be required to take a drug test and/or a criminal background check for employment with JobHook. I understand that if I fail either test, I will be ineligible for employment with JobHook. I further agree to, and here
by authorize the release of the results to JobHook. JobHook reserves the right to repeat test during employment randomly or if illegal drug or alcohol use is suspected while on the job. I understand that if I refuse consent, I will be removed from further consideration for employment.
I understand that if I do have an injury, that I will be required to do a post-accident drug test. No exceptions!
I agree that I will work with JobHook 520 hours before discussing permanent employment. I agree not to discuss or accept employment with any company to which JobHook has assigned me.
I authorize JobHook to contact all former employers for references and release the information received and contained on this application to the clients of JobHook for employment screening purposes. I authorize JobHook to verify my past employment and education, criminal records including sexual predatory offences, credit history if applicable, motor vehicle records, personal references and appropriate individuals, companies, institutions or agencies to release information, and I release the agencies and their employees from any liability as a result of such inquiries or disclosures. JobHook may generate a consumer report summarizing this information. I also authorize JobHook to disclose My Consumer Report to any company where I am, or could be, assigned for employment.
I hereby certify that all information given by me to JobHook is true and correct to the best of my knowledge. I understand and agree that any false information, misrepresentations, or omission of facts in this application and the application process may be justification for refusal to hire, or immediate termination of employment.
Paychecks can be picked up in our office Fridays 8 a.m. to 4 pm. You may select direct deposit. I agree that if I chose to have my paycheck delivered to me for any reason, identification must be shown when picking up checks or if someone else picks up the check with prior authorization. If I choose to have my check mailed, I will accept full responsibility for the loss, theft, Bank charges and any costs for replacing said check.
It is the goal of JobHook to provide a work environment free of discrimination, which includes freedom of harassment; whether that harassment is based on sex, age, race, national origin, religion, sexual orientation, marital status, or membership in other protected groups.
JobHook prohibits harassment of its employees in any form by supervisors, coworkers, subordinance, customers, or suppliers. Such conduct may result in disciplinary action up to and including dismissal of the employee. With respect to non employees, offending customers and suppliers, can result in employee being asked to leave and not to return. No supervisor shall threaten or insinuate either explicitly or implicitly. that any employee's submission to or rejection of sexual advances will in any way influence any decision regarding that employee's employment, evaluation, wages, advancement, assigned duties, or any other condition of employment. Any harassing conduct in the workplace, whether physical or verbal is prohibited. This includes slurs, jokes, or degrading comments concerning sex, age, race, national origin, religion, sexual orientation, marital status, or membership of other protected groups, offensive sexual flirtation, advances or proposition, continual or repeated abuse of sexual nature, graphic verbal comments about an individual's body, and the display in the workplace of pornographic objects or pictures. Employees who have complaints of harassment she report such conduct to JobHook. Employees who observe harassment should also report such conduct to JobHook. All complaints of harassment will be promptly and confidentially investigated.
JobHook is committed to providing and promoting a safe and healthy workplace for our employees, preventing accidents, injuries and illnesses is our primary objective. When an employee is injured on the job, JobHook will use our return to work process to assist the employee in returning to work as soon as medically feasible. We will arrange for immediate, appropriate medical attention for employees who are injured on the job. We will attempt to create opportunities for them to return to safe, transitional work assignments as soon as medically possible. The process may have different names like returned to work, modified work assignment, transitional work, however, our goal remains the same, to return injured employees to safe work. Our ultimate goal is to return our injured employees to their original jobs. If an injured employee is unable to perform all the tasks of the original job, JobHook will make every effort to provide a transitional work assignment that meets the injured workers capabilities. The success of this process involves the combined efforts of management, employees, our designated medical providers and our workers compensation insurance.
