TERMS & POLICIES
* I understand I can reschedule my private care once at no extra charge . I agree to pay additional deposit fees if I request to reschedule my private care more than once within 6 months of first scheduled date.
* I understand if my surgery is canceled due to a medical reason,I will be able to request a reschedule or refund of monies paid minus the deposit.
* I understand Mermaid Island Post Op Retreat will not give nor change my medications. I agree my caretaker will only remind me to take my medications. I agree to only take medications as prescribed by my surgeon for pain.
* I agree to provide an emergency contact name and number.
* I agree to inform Mermaid Island Post Op Retreat of any food and medication allergies
* I understand that special diets such as pescatarian(fish, shrimp, crab, lobster) etc can be accommodated at an additional charge of $150 and up. Specialty drinks such as vitamin water, Ensure, protein shakes, smoothies etc can be purchased from us or ordered from Instacart. I will inform staff when an order is being placed.
* I understand that outside vendors such as massage therapists, IV drip providers, faja providers etc are not allowed at our airbnb. Only approved vendors are allowed. I will verify with staff if my chosen provider is allowed before booking.
* I uderstand that I must prepay for In house massages
* I understand that mail and packages are not allowed to be shipped here to the home.
* I understand that after surgery, disposable sheets, pillowcases, washable bed pads and disposable chucks are placed on the bed to protect the mattress from fluids that leak from my body. If I remove any of the items and blood gets on the mattress because of refusal to properly lay on the barriers, I will be charged $350 for the twin size mattress and $575 for the queen size mattress if using the private suite. These are the costs of replacing the mattresses.
* I understand that lights & TVs must be turned off when exiting a room. TVs must be turned off when going to bed and kept at a reasonable volume as to not disturb other guests.
* I understand that If leaving the home, the return time is 8 PM.
* I undertsand that If I'm using delivery services such as Uber, Lyft, Grubhub etc, i will inform someone on staff. Any deliveries after 8 PM or before 8 AM will be refused.
* I understand that laundry services comprises of washing of surgical garment, compression socks, towel and robe only. Laundry services are not available to companions beyond having their towels laundered. Laundry services begins at 10 AM and ends at 5 PM daily. Only 1 laundry load per guest daily. We use Lysol Laundry Sanitizer, Peroxide and Perisil detergent. If I am allergic or use a specific brand, I will provide it.
* I understand that only toilet paper can be thrown in the toilet. Wipes must be disposed of in the trash regardless if they're flushable. If I throw wipes are in the toilet and it becomes clogged, I understand i will be charged a $250 fee, which is the price of a plumber to unclog the toilet.
* I understand that In case of a medical emergency, I cannot refuse medical attention.
* I agree to respect other guests' privacy. We maintain a peaceful and quiet environment. No loud music or conversations on speaker phones are allowed.
* I understand that no vehicles of any kind are allowed on the premises.
* I understand that any disrespect, fighting , disturbance with other guests or staff will not be tolerated and will result in removal from the premises with assistance from local authorities.
* I understand that there will be absolutely no guests allowed at the home at any time including being dropped off, picked up by family member/friend or having items dropped off to the home.
DISCLOSURES: I understand that Mermaid Island Post Op Retreat is a non medical home care post operative service. I understand that skilled nursing services are not provided. I agree that care will not be provided at a medical facility, medical spa, clinic or by a doctor and that Mermaid Island Post Op Retreat provides care by licensed home health aides, CNA’s and certified body sculpting practioners under Florida Law in a private home setting . I understand my practioner will work within their scope of practice for services provided . I agree that Mermaid Island Post Op Retreat is not responsible for my post surgical results . I agree body sculpting sessions requested at Mermaid Island Post Op Retreat is for the basic purpose of improving lymphatic flow, detoxing toxins and relaxation. I agree to communicate with my practitioner on pressure during sessions. I understand toxins are released through my lymphatic system and my practioner can not force healing. I agree I've been honest with my medical background . I understand Mermaid Island Post Op Retreat is not a doctor nor treats, prescribes or diagnoses medical conditions nor can guarantee post op body results
