The Person responsible for the account remains responsible for any fees according to the School's Financial Agreement. It is further agreed and accepted that this form is only signed once the Person responsible has read, and fully understood, the School's Financial Agreement.

The information is needed to register and entered on the SwitchedOnEducation digital program and the school management system and used to contact you and will not be divulged to any other third parties.
