  • Blood Pressure (BP) *

    Please enter your recent systolic (upper) and diastolic (lower) blood pressure.

    Total cholesterol, HDL cholesterol, LDL cholesterol and triglycerides are REQUIRED.

    If you have results of an Extended Lipid Profile, please enter those results as well for a more accurate risk assessment.

  • Blood Sugar

    Entering the results of the blood tests for diabetes is OPTIONAL, but highly RECOMMENDED if they are available.

  • Electrolytes & Blood Chemistry

    Entering these values is OPTIONAL, but highly RECOMMENDED if they are available, especially if you are taking BP MEDICINES.

  • Liver Function Tests & CPK

    Entering the liver enzymes results and CPK muscle enzyme results is OPTIONAL, but highly RECOMMENDED if they are available, especially if you are taking CHOLESTEROL MEDICINES
