  • ACVR Radiation Oncology Residency Program Annual Update

  • Purpose:  To provide current information to the ACVR and the Radiation Oncology Residency Standards and Evaluation Committee about the status of each radiation oncology residency training program. We also ask that you take a moment to review the Radiation Oncology Residency Program Essentials, Training Standards and Requirements.

     

    The form should be completed by the radiation oncology residency director by January 31st of each year.

     

    When filling out the application, the first full page of information is entered and you hit next, the form will automatically save the information entered to the emailed link for the submitter to stop and start the application with the emailed link. Once a page is started it must be completed and next hit for that page to save.

    Please list only those dates of the actual training program.  Time spent by the resident at your facility prior to beginning or following the completion of the actual training program should not be included.

  • The following questions relate to the status of required faculty in the residency training program. If there is a change in status of any of the required faculty, please state in the box indicated how this change impacts residency training and compliance of your program. Specific reference should be made to the numbers of weeks per year a given faculty member or class of faculty members (e.g. medical oncologists) is available to the resident on a daily basis.
