By submitting this application I agree to and understand:

+ Maintenance appointments with stylist or costylist are recommended every 6-8wks in order to maintain the best results as well as the integrity of the natural hair

+ Stylist will provide extensive written and verbal post care instructions I am expected to follow in order to maintain optimum results

+ a $500 non-refundable retainer fee is required to reserve your appointment

+ Pricing includes hand-tied extension hair, color, treatment or gloss on natural hair, installation, and cut to blend

+ Installation price can range from $1400-3000, and any remaining balance must be paid on date of service

+ If a no-call, no-show occurs, I will no longer be eligible to reserve an appointment in the future
