  • Home Burial

    You may take your pet's remains home for burial at no charge. This option does not include memorial items.

    Communal Cremation

    You may have your pet's remains cremated for $50. Your pet's ashes will not be returned to you.

    • This option does not include a clay paw print. You can request clay paw print for an additional $40/each.
    • You can request a nose print for an additional $20/each.
    • You can request a hair clipping for an additional $20/each.

    Private Cremation

    You may have your pet's remains cremated for $175. You will have the ashes returned to you.

    • This option does include 1 clay paw print. You can request additional clay paw prints for $30/each.
    • You can request a nose print for an additional $20/each.
      You can request a hair clipping for an additional $20/each.
    • Urn selection: 
      • You can select one of the 3 included urns shown below for no additional cost. To select an urn not listed, or to customize the urn with a nameplate, you can follow this link:  Memorial Products .  (Note: Our cremation service, Fond Memories, will contact you via phone for payment on specialty urns and products.)

    •      I, the undersigned, certify that I am the owner (or the duly authorized agent for the animal described above, and consent to the humane euthanasia of this animal. 


           I give Lake Austin Boulevard Animal Hospital full and complete authority to euthanize my pet in a humane manner and in accordance with the rules and regulations of this establishment. 

           Furthermore, I release the veterinarian, representatives, and the clinic from any and all liability of said euthanasia. 


           I understand that euthanasia is the act of ending the life of an animal in a painless way to prevent unnecessary suffering.
