Step Up Scheme Referral Form

Referral Form
The Step Up Scheme provides 6, one hour sessions of Activity Based Rehabilitation for individuals who have been diagnosed with a spinal cord injury in the last year. These sessions are provided at no cost to the participant, all costs are covered by Neurokinex through fundraising and the generosity of donors.
