I, the client/model as herein signed, authorize Tantalizing Tresses, to perform hair, makeup, waxing, & any other beauty services to needed for desired outcome. These services include, but are not limited to shampooing, conditioning, treatments, chemical coloring, chemical straightening, steaming, detangling, blow drying, trimming/cutting, clipper cutting, thermal straightening/curling, facial waxing, facial hair tweezing, makeup application, photograhy, videograpghy, etc... I understand these services require me to follow the directions of the Licesenced Cosmotelogist and staff. I have been fully informed as to the methods and techniques concerning these services. The known risks of the cosmetic, chemical, and/or thermal services that have been chosen, have been disclosed to me. Some cases my result in complications. It has been represented to me that no guarantees, warranties, promises, commitments or other statements as to the results of this service have been made, and I acknowledge that I have received no particular representations or guarantees, and I am consenting to these services at my own risk. I have revealed or disclosed all conditions and circumstances regarding my health and hair history, medications being taken, and any past reactions to products used. Additional conditions may occur or be discovered during or after the services, which could affect my ability to tolerate future services.* This field is required.

I, the client/model as herein signed, release, give up, acquit, and discharge Tantalizing Tresses & staff or any affiliates including any partnership, corporations, or company associated with said individual from any claims or damages of any nature. I agree to pay any costs of legal services necessary to further affect or confirm said release.
