I have read the above information and have given an accurate account of the questions and if I have any concerns, I will address these with my service provider. I give permission to Brenda, to perform the waxing procedure we have discussed and will hold her harmless from any liability that may result from this treatment. I agree to adhere to all safety post care including: no peels, tanning, or wet room services; no swimming/spas/hot tubs for 72 hours after waxing; and all home skin care protocols as recommended by my service provider. I understand that my wax specialist will take every precaution to minimize or eliminate negative reactions as much as possible.
POLICIES

CANCELLATIONS: At least a minimum of 24 hours notice to cancel or change an appointment. If you cancel with less than 24 hours’ notice, you’ll be charged a 50% fee of your service.

LATE: If you will be 5 minutes late (PLEASE LET ME KNOW) by 10 minutes, your appointment will be canceled and you will be charged 50% of your service. You will have to reschedule online.

NO SHOW: If you do not show up for your appointment, you will be charged 100% for your full service. No exceptions.

Second time no show, you will be blocked from making any future appointments
