I give my permission to Nancy Bay to seek emergency medical treatment for my pet as deemed necessary by a Verterinarian and will be responsible for payment of such care. I understand that Nancy Bay will make every attempt to contact me first. 

I understand and acknowledge that Nancy's Pet Care Service is a small privately owned pet service hereby absolved of any liabilites regarding any/all pets that are within her care. I do realize that Nancy Bay will do everything within her power to provide a loving, safe and clean environemnt.
