This form is for all High School Youth (grades 9-12) currently enrolled in our Year 1 & Year 2 Confirmation Programs.

As per our Year 1 Confirmation requirements, candidates must attend, participate, and reflect on at least three (3) HSYM nights. Please check off which HSYM program (in-person or virtual) you plan to attend this semester. This will allow us to share the appropriate resources with your family.
