    • The NewboRN Home Visiting Program (NHVP) is a universal home visiting program that is free and offered to all Alachua County women who give birth at a hospital, birth center or home within the county. Within a week after baby is born, a registered nurse will visit the family at home to see how they are doing, share information and connect to services and resources. The nurse supervisor is responsible for providing supervision, reflective practice, staff development, support and oversight to the NHVP nurses. The nurse supervisor will also be responsible for conducting home visits and assessments on program participants.

    • This position with the Connect-Coordinated Intake and Referral (CI&R) program provides initial intake, outreach, case management services to pregnant women and families with young children, and education on family planning. This position requires strong telephone communication skills, and basic knowledge of Medicaid, pregnancy issues and home visiting.

    • This position is responsible for planning and conducting outreach and trainings to prenatal care providers, delivery hospitals, birthing facilities, and pediatricians within the Central Healthy Start Coalition service area (Citrus, Hernando, Lake and Sumter counties) to promote screening and referrals to home visiting programs and educate on the services offered by the home visiting programs.

    • High Impact Prevention Program (HIP) is an approach by The CDC to provide proven disease intervention methods to the populations that need them the most in order to prevent the greatest number of new infections and reduce health disparities. WellFlorida’s HIV High Impact Prevention Program provides HIV prevention and HIV Linkage to Care services to the residents of Area 3/13 (15 county region in North Central Florida). Under direction of the Director of Community Initiatives Department, the HIV Prevention Associate will work with the other members of the HIV High Impact Prevention team to deliver HIV High Impact Prevention strategies, including: HIV testing; community HIV education and outreach; implementation of other evidenced-based initiatives including Business Responds to AIDS, Popular Opinion Leader, and MAPs (linkage and adherence program for HIV positive individuals). This is a grant funded position and is subject to availability of funds.

    • WellFlorida Council is the Ryan White Part B lead agency for Area 3/13, a 15 county region in North Central Florida. Under direction of the HIV Program Director, the Ryan White Medical Case Management Supervisor will help develop WellFlorida’s new case management program, in accordance with Ryan White federal and state guidelines. This position will manage a caseload of clients and supervise other case managers as the program grows. This is a grant funded position and is subject to the availability of funds.

This position must be comfortable with computers, general office tasks, and excel at both verbal and written communication. Most importantly, the Program Assistant should have a genuine desire to meet the needs of others.

    • This position requires health planning and administrative knowledge, skills and ability in the coordination, development, and achievement of major agency projects for both internal and external clients.  General responsibilities include: individual project development and management; assigned management and coordination of project workflow; collecting, analyzing, evaluating, and presenting research information, plans and programs for health system and health policy development and community health improvement; marketing and outreach of WellFlorida products and services for the purposes of cultivating new projects, clients and revenue streams; and regular presentations to key constituencies including government, public and private entities.  This position reports directly to the Director of Community Initiatives.

    • The Associate Planner provides technical and administrative assistance, under supervision of the Director of Community Initiatives, to clients, internal program staff, board members, special interest organizations, members of the community, and providers.  The Associate Planner completes studies, prepares reports, and advises public and private sector administrators on feasibility, cost effectiveness, and regulatory conformance of proposals for special projects or ongoing programs.

    • The Chief Financial Officer (CFO) monitors and manages the organization’s finances as well as the finances of individual programs within the organization; is responsible for all forms of internal and external financial reporting; and oversees and manages all fiscal and administrative support staff. This is a professional position that reports to the Chief Executive Officer (CEO). The position directs the financial management program that encompasses the coordination and integration of functional activities associated with budget, accounting and managerial financial reporting, grants management and the supervision of the internal fiscal processing team. The person holding this position is a member of the Senior Executive Team and works closely with the CEO, the Chief Operations Officer (COO) and Program Managers along with WellFlorida Board members, corporate partners and others on the WellFlorida staff as needed.

The aim of the FIMR project is to enhance the health and well-being of women, infants, and families by improving the community resources and service delivery systems available to these families. FIMR brings together key members of the community to review individual fetal and infant mortality cases then develops and executes strategies to mitigate the circumstances that contributed to the fetal/infant death. The FIMR Nurse Abstractor is responsible for objectively capturing comprehensive quantitative and qualitative data related to the circumstances surrounding identified fetal and infant deaths and summarizing and interpreting medical findings for the rest of the FIMR team, the Case Review Teams (CRT), and the Community Action Groups (CAG).

The Fatherhood Program Coordinator is responsible for delivery of evidence-based father-focused parenting curriculum in community locations, as one-on-one services, recruiting father participants by engaging in community outreach activities and community education, as well as making and maintain partnerships with other community service organizations.  This position is also responsible for providing direct services to support fathers’ involvement through case management, care coordination, fatherhood initiatives, and life course education.
