Client Waiver and Release Agreement
1. It is the dog owner’s responsibility to inform the groomer if their dog has ever bit anyone, including another groomer. I understand that as the owner I could be held liable for any injuries to the groomer resulting from the failure to disclose this information.
2. It is not recommended to tranquilize your dog for grooming. A muzzle or gloves can be used if your dog is showing signs of aggression, however, Fit to Be Groomed will always give them the benefit of the doubt first. I will let the groomer know prior to grooming if I think my dog may bite or has bitten so the groomer can be prepared rather than surprised.
3. I understand Fit to Be Groomed will do its best to protect the health and safety of my dog while on the premises. I acknowledge that accidents, however, can and will happen and I hereby absolve Fit to Be Groomed and its groomer from all responsibility involving injury, escape, damage or disease during and following their grooming appointment at Fit to Be Groomed.
4. I grant my permission for Fit to Be Groomed to obtain emergency veterinary treatment for my dog at my expense should it become necessary.
5. I understand that senior dogs may have underlying conditions that can be aggravated during or after normal grooming procedures. Such conditions may prevent haircut from being completed. Your dog’s comfort and safety is the main priority.
6. I understand should my dog require a complete shave down, there is always a risk of minor nicks and/or irritation when shaving a matted coat. This is due to unseen warts, moles or skin that can be pulled up into the mats. I further understand that when there is heavy matting, moisture can be trapped near the skin allowing molds, fungus or bacteria to flourish, resulting in a variety of skin irritations and conditions.
7. I will make every effort to pick my dog up within a reasonable amount of time once I receive the pick up window notification from groomer.
8. Returned checks are subject to a $25 fee.
