Disclaimer and Consent
This is not psychotherapy/counseling: The Client is aware that the coaching relationship does not represent psychological counseling or any kind of therapy. The Client is also aware that coaching results can vary and are not guaranteed. The Client agrees that he/she/other is entering into coaching with the understanding that the Client is responsible for his/her own decisions and results. The Client also agrees to hold the Coach free from all liability for any actions or results for adverse situations created as a direct or indirect result of advice given by the Coach.
This is not a sexual relationship: The Client also understands and agrees that despite the possible sexual nature of discussions, there is no direct sexual exchange allowed between the client and the practioner. Any interaction that violates this will likely lead to discontinuation of care without reimbursement. The practioner has the right to determine at any time that this agreement has been violated.
Client Session Contact Procedure: The Client shall contact the Coach at the agreed hour only. If it is determined that the Client is regularly contacting the Coach outside of session they will be issued a warning. If the Client does not heed the warning, the pracitioner may cease care options.
Payment: Payment for sessions is due up front prior to the start of the session, after the initial free consult is performed. There are no refunds on any of offered services. If there is a package of visits purchased, the payment for all sessions at the quoted price is due up front. If a package is made specifically for you at a negotated price, this amount will also be due up front.
Cancelation Policy: If the Client fails to make the appointment time, the Client is still responsible for the coaching fee, unless 24 hour notice was given to reschedule the contact. The Coach will make every effort to reschedule with the client.
Missed/Late Appointments: If the Client arrives less than 20 minutes late to a scheduled appointment they will be allowed the remaining time. If the Client is more than 20 min late, the appointment value is charged and the appointment will need to be rescheduled with the practitioner. If the Client is regularly late for appointments this may be cause for dismissal of care.
Emails/Texts outside of session: Emails or texts that take longer than 5 minutes to answer, and do not pretain to scheduling or billing issues, may result in a charge. It is best if the Client saves discussion for scheduled sessions unless the particitioner requests otherwise.
Communication with your medical team: There maybe times when it is benefical to the Client to have the practioner speak with a member of the Client's healthcare team to help improve and coordinate care. Such practitioners may include but are not limited to: physical therapists, massage therapists, nutrionists, psychologists, psychiatrists, primary care providers, sex therapists/counselors. The Client will be consulted regarding this contact prior to any information being disclosed.
Information Disclosure: All of the Client's discussions are private. In specific cases (as defined by the State of Washington) it may become necessary to disclose information about the Client's discussions. These include: cases of strong intent to harm, the abuse of a child, the abuse of an at-risk adult, or when compelled by court order the client's information can be released without permission.
For Physical Therapy/Massage and Live Guidence: The Client will be required to provide medical information to the practitioner and will be required to sign an additional consent form pertaining to these services.
