Please read, understand and sign the following release form.

Spray tanning is accomplished by application of a solution containing the active ingredient: DHA (Dihydroxyacetone DHA is considered to be safe and has been FDA approved ONLY if you follow guidelines to protect mucous membranes. The FDA advises asking the following questions when considering the application of DHA by spraying or misting:

· Are you protected from exposure in the entire area of the eyes, in addition to the eyes themselves? · Are you protected from exposure on the lips and all parts of the body covered by mucous membrane? · Are you protected from internal exposure caused by inhaling or ingesting the product?

If the answer to any of these questions is "no," you are not protected from the unapproved use of DHA. You should request measures to protect your eyes and mucous membranes and prevent inhalation. Please request a mask and / or nose plug at the beginning of your session if you do not have one. Wearing a nose plug is completely at your discretion.

 

NOTICE: you will be standing for approximately 12-15 minutes with your arms up and down, turning side to side, please hydrate yourself and eat something before your appointment. If you are feeling faint or light headed prior to your appointment and need a snack/water, please let your technician know. If you have an sensitivities to smells or fumes, please let your technician know before the session. Any prior medical issues MUST be discussed prior to session. Water is always available if needed!

• You will enter a private room where you will remove your clothing. This is up to you and your level of comfort. If you are a female, you may be airbrushed nude - all males must wear some type of undergarments. Should you decide to wear a swimsuit or personal undergarments, make sure it is dark colored. Spa undergarments may be available upon request for $1.00. The solution will wash out of most clothing. It is always best to wear dark loose fitting cotton clothing after your session. It is advised to wash the undergarment or clothing worn as soon as possible after your session.

• Next, You will be sprayed. This process will take approximately 12 to 15 minutes. After spraying, your skin should be dry before putting your clothes back on and you should not bathe or sweat excessively until you have rinsed off the bronzer layer at the appropriate time based on your solution choice. The solution will give you an immediate bronzing effect. The bronzing effect is a result of a coloring additive in the solution that will remain on the skin until you are actually tan. When you shower, the coloring will come off to reveal your actual tan beneath, OR if it is a Express/Flex tan it may take 8-12 hours before the full tan id developed.

• All skintypes are different. All ingredients used in this procedure are intended for cosmetic use and generally regarded as safe. There are, however, occasions where individuals may be allergic to one or more ingredients in the spray tan solution. Please read the ingredients list if you have any known allergies - Ingredients in our solution include Citrus, elderberry extract, caffeine, white tea extract, green tea extract, jojoba seed oil, aloe, and Chamomile, etc. The technician will be more than happy to share ingredients and provide a swatch test.

• Be advised there is a small percentage of people whose skin may not react favorably to spray tanning. For this reason, we do NOT advise being sprayed for the first time when your appearance is critical; (wedding/special occasion)

• Caution – Pregnant or nursing women should consult their physician before using.
• Warning – This product does not contain a sunscreen and does not protect against sunburn. Repeated exposure of unprotected skin to U.V. Light may increase the risk of skin aging, skin cancer and other harmful effects to the skin even if you do not burn.

• If you have had a sunburn in the last 1-8 weeks and it has resulted into peeling or has had severe redness, we do not recommend getting sprayed but it is at your discretion to go through with the service. Spraying over sunburned skin/healing skin/peeling skin can result in dryer skin causing uneven tans, patchy or peeling tans. Please be advised that this can happen no matter how severe the burn is/was, and that we DO NOT recommend getting sprayed until skin is fully healed. If the tan does not develop favorably due to sunburn/old sunburn, it is not the technician's fault. If you arrive to your appointment with a sunburn and decide not to go through with the appointment because of the results, there will be a late cancellation fee of $25.00 to cancel the day of. Please protect your skin from sunburns if you have an appointment booked. 

• If you do not properly protect or prep your skin before your tan or properly maintain your tan after your session, it is NOT at the fault BRONZED SYRACUSE, LLC for any streaking, patching, or uneven tans once you leave the service provided by the airbrush technician. The technician will discuss all aftercare and maintenance directions during and after session. A care card will be provided after each session, if requested. I (the client) have been provided with spray tan prep and care instructions, which I have read and understand completely. To my knowledge, I have no medical condition or allergy which would preclude me from going through with airbrush tanning sessions with BRONZED SYRACUSE LLC. I have been honest and accurate about the information that I have provided on this waiver. I take sole responsibility of any reaction I may have, staining of clothing and/or personal belongings.

I (the client) am soley responsible for proper preparation for the appointment, as well as proper aftercare and maintenance of the tan. If my results are not favorably due to lack of prep/maintenance, following aftercare instructions, sunburns, dry skin, underarm/underboob sweat, etc. it is not at the fault of BRONZED SYRACUSE LLC.
