• Schlow to School Program Signup Application: Student

    To participate in the Schlow to School program, please fill in this form. Items are delivered twice a week during the school year to your school. Deliveries are suspended during all breaks. Fines are not assessed; however, the student and parent/guardian are responsible for all long-overdue/lost or damaged materials. If you need help completing the form, contact your school librarian.
  • How does the Schlow to School account work? 

    • Initial checkout period of 4-weeks, with the possibility of two 4-week renewals allowed if there are no current reserves on the item. 
    • Email notification of reserves, items coming due soon, and overdue items sent to the email address provided. It is up to the student and parent/guardian, to provide updated email account information.
    • Although Student Delivery accounts do not accrue daily overdue fees, the material is expected to be renewed or returned by the due date. All accounts will be billed for replacement costs for long-overdue items considered lost.
    • The following items are not eligible for shipment. They are still available for checkout but must be picked up and returned at Schlow Centre Region Library: musical instruments, equipment.
    • Parents/guardians determine whether a student will participate and are responsible for notifying the school if the student either transfers to another school or wishes to cease participation. 
    • The Schlow to School Delivery Program is a special service and will be revoked if misused, such as not returning or renewing material by the due date. 
    • Once activated, participation in the Schlow to School Library Delivery Program continues through one school year. Summer ends the contract period during which time, students will assume standard loan periods/fines. Students must fill out a new participation agreement at the beginning of a new school year.  
  • The student/parent/guardian agrees to pay any costs or fees that may be charged for materials, lost or damaged. The student/parent/guardian agrees to notify the school librarian if the student transfers to another school or no longer wishes to participate in this program. The responsibility for the choice of materials borrowed rests with the person checking out the material, not with Schlow Centre Region Library or school staff. The student/parent/guardian understands this account is restricted to borrowing of materials related to the student. Participation in the school delivery program is a privilege that can be revoked if abused.
