PLEASE READ ALL INSTRUCTIONS CAREFULLY: This registration and payment form is for the Artworks location at 1840 Nacogdoches Rd, SATX 78209. All camp payments are non-refundable. If you ignore the waitlist instructions and make a payment for a FULL / WAITLISTED Camp Artworks is entitled to keep your payment and issue you a credit for the future. Please read Liability Release Waiver (SEE BELOW) before submitting this form. Submission of this form is a valid signature.
PLEASE MAKE SEPARATE REGISTRATIONS FOR EACH CHILD; A $20 Registration Fee will be charged for unregistered children at the time of drop-off.

$75 SUMMER SATURDAY Morning CAMPS: MORNING HALF DAYS ONLY 9am-12pm - These Morning Camp Sessions will feature a FINE ART INSTRUCTED PROJECT in a SMALL GROUP Setting (no more than 10 students). Each Student will bring home a "Finished Product" as well as their own free-time creations. An optional snack will be provided. Best for Ages 6+

LIABILITY WAIVER AND RELEASE FORM (MINOR CHILD)
THIS IS A RELEASE OF LEGAL RIGHTS — READ AND UNDERSTAND BEFORE SUBMITTING THIS FORM!

I hereby certify that I am the adult parent or guardian of the child described above, a minor child under the age of eighteen years, and I consent to his/her participation in recreational activities at an art studio, (Artworks Art Studio) and his/her use of art material & tools provided by the art studio instructors.
I understand and acknowledge that I am fully aware of and assume the risks (including but not limited to the risk of serious bodily injury, property loss or damage), of (1) said minor child’s participation in recreational activities at the
studio, and (2) his/her use of the art materials & tools. I recognize my responsibility to ensure that said minor child participates only in
those activities for which he/she has the required skills, qualifications, training and physical conditioning. I understand that Artworks Art Studio shall have no responsibility to pay for medical treatment and related costs if said minor child is injured.

Knowing the risks described above, I agree, personally and on behalf of the minor child named above, to assume all the risks and responsibilities surrounding my minor child’s use of the Studio and materials/tools. To the fullest extent allowed by law, I hold harmless and agree to indemnify Artworks, its officers, directors, faculty, staff, volunteers, employees and agents, from and against any present or future claim, cause of action, loss or liability for injury to person or property, which said minor child may suffer or for which said minor child may be liable to any other person, related to said minor
child’s participation in recreational activities at the Studio and use of the materials/tools, resulting from any cause whatsoever, and regardless of fault.
I am at least eighteen years of age and have carefully read and freely signed this Liability Waiver and Release Form(Minor Child) by way of submission for registration. I understand and agree that no oral or written representations can or will alter the contents of this document.

Refund Policy: ALL CAMP PAYMENTS ARE NONREFUNDABLE! If you accidentally ignored the instructions and made a payment for a waitlisted camp please contact us before 5pm end-of-day on the same day of your payment to void your payment without penalty. Artworks is entitled to keep your waitlist payment as a credit for future camps.

Structure of a Camp Day: Campers can enjoy studio free-time between activities throughout the camp day as long as they have finished projects, followed instructions, and cleaned up! Free-time activities include, painting, playdo, free-draw, library books, building with toys, or any hand-craft we have available in the studio that day.
