In consideration of voluntary participation in the Wylie High School Band Booster Club (hereafter referred to as "Booster Club"):
* I hereby release and hold harmless the Booster Club, its officers, and members for any loss or claim that may be sustained in association with participation in activities of the Booster Club. This includes both on-campus and off-campus activities, trips, practices, etc.
* I agree to abide by all the Wylie Independent School District, Wylie High School, and Booster Club policies and procedures as they pertain to parent and/or booster volunteers, including but not limited to, policies outlined in official policies of the Wylie Independent School District, Wylie High School, and the Booster Club.
* I understand that this Volunteer Application shall not constitute any obligation by the Booster Club to assign a specific volunteer task or opportunity. The selection of volunteers shall rest under the authority of the Booster Club President, designee, or the Band Director.
* I acknowledge that I willingly agree to be subject to a Texas Department of Public Safety sex-offender registry check(s), as outlined in the Booster Club policy for volunteers. Any failure of background checks will result in a temporary and confidential suspension of my Booster Club volunteer status, pending any optional appeal to the Booster Club Executive Board, while I pursue correction of unfavorable, erroneous, or false-positive results.
*** If signing up to be a chaperone, I acknowledge that there will be a more extensive background run at the expense of the Booster Club. This is only for chaperones who travel with the students or will be directly supervising the students.
In consideration of my good faith attempt to volunteer, a failure to pass any background check will remain confidential indefinitely. Background check outcomes shall be valid for one year.
