I understand that I must be licenced in order to take part in regular training with UKTI, and I wish to become a Licence holder of your organisation.
PLEASE READ:
BEFORE the 3rd lesson - UKTI require a ‘one-off £45.00 Licence activation fee’ if choosing to pay by the monthly direct debit, which is an initial payment which activates a student’s ongoing licence whilst training at any UKTI class location, it also enables a direct debit holder’s licence to be renewed automatically each year free of charge as long as the direct debit remains active. This form must also be submitted followed by the payment BEFORE the 3rd lesson. After you submit this form, you'll see a button to pay the 'one-off Licence Activation fee' right away. Once monies are tendered it is non-refundable, and non-transferable.

• Licence book - You will receive the Licence and Grading record book after your first Course & Exam.

• Licence - The applicant will be licensed for regular training at any club conducted by UKILT and approved events and seminars organised by UKILT or recognised 3rd parties. Regular training with 3rd party organisations apart from us will not be covered. Please contact our head office if you are unsure of cover: uktienquires@gmail.com We reserve the right to refuse any Licence without explanation and our rules are subject and can be changed at any time.

• Medical Disorders - Applicants who suffer from any medical disorder must attach a letter to us by email immediately after submission of this application to: uktienquires@gmail.com with written confirmation from your doctor stating that the student is fit to practice physical training & Martial Arts. Failure to do so may invalidate your Licence.

• Database - How we process your data can be found here: https://www.ukti.info/privacy

• Physical Contact - Applicants should be aware that from time to time it might be necessary for the Instructor to make physical Contact with the applicant to adjust posture. Sparring and Self-Defense can also result in physical contact between students.

• Photography - Photographic/video equipment will be in use during our classes & events. Images Captured may be used for Promotional and/or training purposes, parents/guardians are NOT allowed to use mobile phones during the class times.

• Uniformity:
To Comply with official requirements, all members must wear the same uniform at all times & only use UKTI approved equipment. It is therefore essential that all students speak to their instructor before purchasing any equipment.

1. I accept that the practice of a Martial Art may involve the risk of injury. 2. I have never been convicted of a violent crime. Please Note: All monies once tendered are non-refundable and non-transferable. I apply for my UKTI Licence and agree to comply with the rules and regulations of the organisation. These rules are subject and can be changed at any time.
