    Parent Acknowledgment and Attestation Form

    All students are required to complete the Parent Acknowledgment and Attestation Form for bus transportation of any kind for the school year. (Includes daily transportation to and from school, field trips, athletic, band, and any other bus transportation for school events or programs).

    Thank you for your patience and cooperation as we make the safety of our students, staff, and community our top priority.

  • Transportation Services Parent Acknowledgment and Attestation Form

    It is the intent of Carter G. Woodson School to provide effective transportation services for all students assigned to ride a school bus.  The following information from the NC Department of Health and Human Services provided prior to requesting transportation services for 2020-2021 school year.

     

    • CGW will require face coverings for all student riders that are able to wear them: inevitably, there will be a student(s) on the bus that will not be able to wear a face covering because the student(s) have a qualified exception.  By signing up for bus transportation services, I understand that my child(ren) may be in a seat next to a student without a face covering.
    • Except for students from the same family, students will be required to sit one to a seat on the bus.
    • Student health screening activities will occur at the school, not at the bus stop.
    • School buses will be cleaned in the mornings after all students are transported to school and again in the afternoon after all students are delivered home.  Drivers will clean handrails between all runs.
    • Strategies intended to ensure effective and on-time transportation service are as follows: 
      • Consolidation of stop locations into community stops where possible (resulting in more direct routes, less ride time and fewer cul de sacs/neighborhood streets traveled).
      • One-stop location per student (keeping students grouped together as much as possible from day to day as recommended by the NC Department of Health and Human Services)
    • Parents are encouraged to provide transportation to and from school when possible.
       

    By signing up my child for school bus transportation services for the 2020-2021 school year, I acknowledge and accept the above risks and protocols.

    • I agree to take my child’s temperature daily before putting my child on the bus.
    • I agree to keep my child home if he or she has a fever, has had close contact in the last 14 days with someone diagnosed with COViD-19, or has any of the following symptoms: chills, shortness of breath or difficulty breathing, new cough, new loss of taste or smell.
