I certify that all written and oral statements I have submitted are true and I understand that false or incomplete statements are grounds for dismissal. I authorize my former employers, other persons and organizations to provide current and accurate information about my background. I understand that my employment with Inline Tube and continuation of employment is dependent upon the information I have submitted. If employed, I understand that if I am hired, upon employment, my employment is "at-will." This "at-will" relationship can only be modified by an express agreement signed by both myself and an Officer of Inline Tube. I understand that I may be assigned work on any shift and overtime is required.
Michigan law requires employers to make accommodations to handicapped applicants and employers where the accommodation does not impose an undue hardship on the employer. Handicapped employees and applicants may request an accommodation of their handicap by notifying the Co. in writing of the need for accommodation within 182 days of the date the handicapped person knows or should know that an accommodation is needed. Failure to properly notify the Co. will preclude any claim that the employer failed to accommodate the handicapper.
