Boarding of your dog is putting him/her at risk for contracting a contagious disease due to close confinement. Serenity Animal Hospital & Pet Resort will not be held financially responsible should your pet contract a contagious disease while boarding since that is a risk you assume. This includes contraction of intestinal parasites. *

However, please be advised that animals may damage or ingest belongings when left alone overnight in the kennel. I agree that Serenity Pet Resort is not responsible for damaged or lost belongings. Nor, will Serenity Pet Resort be held liable for your pet ingesting any toys or blankets. *

Occasionally, boarding your pet may cause additional stress and this may cause your pet to not eat very well. Boarding your pet can cause weight loss associated with stress, even if you pet is eating his/her normal amount of food. Would it be ok to add canned food to your dog's food to entice him/her to eat?*

If your pet is still not eating with canned food offered, we have an appetite stimulant that can help encourage your pet to eat. The cost of this medication is $6 per day. Non-Clients of Serenity Animal Hospital will need to have their pet examined by a doctor on staff and will be charged an exam fee of $49.50 prior to starting the medication. Would it be ok to administer Entyce to your pet if he/she is not eating?*

Boarding your pet can cause stress-related diarrhea, muscous-like stools or gas. We can provide comfort to your pet by adding in a daily probiotic as needed. Probiotics can aide in easing any discomfort your pet may be experiencing. Can we add Fortiflora at $1 per dose to your dog's food if necessary?*

Boarding can also cause dogs to become anxious or fearful. Unfamiliar surroundings, smells and sounds can lead to your pet experiencing stress. We are offering an all-natural supplement called Composure Pro that has many benefits for your dog. It helps enhance your dog's overall mood, promotes relaxation and the ability to deal with external stressors without causing drowsiness. While we highly recommend Composure Pro for anxious pets, the benefits will surely assist any dog staying in our facility. The cost of Composure Pro is $1 per dose. Please initial after reading: I understand that animals which get along at home may become aggressive towards one another when placed in the same run or cage. Should your pets become aggressive towards each other, they will be separated and the boarding fees adjusted accordingly. Should any injuries results from the aggression, the injuries will be treated at the owner's expense. I agree not to hold Serenity Pet Resort responsible for any injuries that arise as a result of my pets being boarded together. *

IF PAYING FOR EXTRAS- We always try to complete the extras you sign up for, however, sometimes things can get in the way like weather, staffing issues, pet's disinterest and so on. If your pet is not interested in the extra you wanted, would you like us to try alternative extras? (Please note that any extras not completed for any reason will be removed from your bill)

Special Attention Pets- We pride ourselves on having staff members that are willing to work with all temperaments of animals. It is very rare we turn away a pet entirely due to behavior, but do understand that if we end up having to schedule our staff around your pet, there will be an additional $15 per night fee. I understand that if my pet requires special attention like stated above, I may be subject to this fee.

IF YOUR PET IS STAYING WITH US FOR A PROLONGED PERIOD OF TIME : We have no issue with long-term boarding pets, but please understand that we require a deposit to reserve your pets room for any stays longer than 2 weeks. If the pick-up date is uncertain, we may require a larger deposit. For extended stays we can provide a weekly payment plan.

I understand that if my pet is staying for 2 weeks or more I will be subject to pay a deposit.

I agree to respect Serenity Pet Resort's hours of operation, and I will honor the times and dates I reserved to the best of my ability. If you find yourself needing to pick up at a later date or time due to outside factors, we will do our best to accommodate you, but please understand we may not always be able to depending on our availability. Additionally, if you are picking up or dropping off outside of our typical hours of operation there is a $10 inconvenience fee that will be added to your bill. Our hours of operation are 8:30a-5:30p Monday through Saturday, and 8:30a, 2:30p, and 5:30p on Sunday/Holidays. PLEASE INITIAL ONCE YOU HAVE READ THIS.
