Only BP Church members may submit this form, and requesting member or designee must be present to host for the event's duration (including recurring events). Requesting specific space(s) does not guarantee use of that space. Space is assigned based on church event priority, size of group, type of activity, and the needs of other requestors.If using the kitchen, Kitchen Coordinator Carolyn Hutton will connect with you prior to your event. Once event is over, immediately complete a kitchen clean-up checklist (located on the side of the kitchen fridge), and place in the "Completed Kitchen Checklist" slot. Please be aware that children under 12 are not allowed in the kitchen without direct parental supervision.For personal events, member provides all consumable paper and plastic products.If an event is catered, catering company must supply, use, and remove all of its own equipment and event trash. On-site storage is not permitted.Before Leaving: Turn off all lights and appliances; lock all windows and doors. Also ensure that no water is running, that trash cans are not overflowing; that toilets have been flushed, and that all exterior doors are locked and secured. If last one exiting parking gate, lock gate and spin combo.A/C & Heating: Please do not adjust the thermostats as it interferes with their pre-programming. The thermostats will be set in advance and programmed to meet your needs at the time of your event. Keep windows and doors closed when in use, restore original settings before leaving, and report issues promptly.More than one group may be using different areas of the facility at the same time. Your group’s activities are limited to the area(s) approved and the restrooms.All set-up and clean-up activities, including decorating, must be completed within your "Actual Time." Decorations must be tasteful, cause no damage, and leave no residue.Check bathrooms for garbage overflows, flush toilets, ensure there is no water running in kitchen or bathrooms.Furnishings Remain Inside During Events: No items belonging to the church may be taken outside or borrowed without prior consent of the Office Staff (and by approval of Facilities Manager and/or pastor or designated elder).Building access: If necessary, post someone at a locked door to allow your guests inside; never prop a door open and leave it unattended.Personal Items/Valuables: The Church is not responsible for lost or damaged personal property. Please monitor personal items.If your access requires a key, please contact the church office to make arrangements.Cancelation(s) & Changes: If at anytime you need to cancel a date or make a change, please contact the Church Office at (360) 892-4664 or email office@bpchurch.com.

Insurance, Waiver & Indemnification: User agrees to hold harmless, indemnify and defend Brush Prairie Baptist Church (including Church’s officers, employees, and representatives) from any and all liability for injury or damage including, but not limited to, illness, exposure to infectious or communicable disease, bodily injury, personal injury, emotional injury, or property damage which may result from any person using the above described premises, its entrances and exits, and surrounding areas, for User’s purposes, regardless of whether such injury or damage results from the negligence of the Church (including Church's officers, employees and representatives) or otherwise.

Claims/Disputes: Any claim or dispute arising from or related to this agreement shall be settled by mediation and, if necessary, legally binding arbitration in accordance with the Rules of Procedure for Christian Conciliation of the Institute for Christian Conciliation. Judgment upon an arbitration decision may be entered in any court otherwise having jurisdiction. The parties understand that these methods shall be the sole remedy for any controversy or claim arising out of this agreement and expressly waive their right to file a lawsuit in any civil court against one another for such disputes, except to enforce an arbitration decision.
