If USING CHURCH FACILITY AND/OR EQUIPMENT (Other Than Vehicles), Please Read and Check Each Box Indicating Agreement:

Requesting specific space(s) does not guarantee use of that space. Space is assigned based on church event priority, size of group, type of activity, and the needs of other requestors.More than one group may be using different areas of the facility at the same time. Your group’s activities are limited to the area(s) approved and the restrooms.All set-up, decorating, activities, and clean-up must be completed within your "Actual Time."Requesting member is responsible to reset space(s): put away tables, chairs, and equipment. Also, please vacuum, check bathrooms for garbage overflows, flush toilets, ensure there is no water running in kitchen or bathrooms, turn off all lights, lock all windows, doors, and parking gate.If using the kitchen, Carolyn Hutton will connect with you prior to your event. Once event is over, immediately complete a kitchen clean-up checklist (located on the side of the kitchen fridge), and place in the "Completed Kitchen Checklist" slot. Please be aware that children under 12 are not allowed in the kitchen without direct parental supervision.Please do not adjust the thermostats as it interferes with their pre-programming. I have read and understand the BRUSH PRAIRIE BAPTIST CHURCH MINISTRIES WAIVER Liability: REQUESTOR AGREES TO ASSUME FULL RESPONSIBILITY FOR INJURY TO PERSON AND DAMAGE TO PROPERTY ARISING FROM THE USE OF FACILITY AND/OR EQUIPMENT AND/OR VEHICLE. REQUESTOR HEREUNDER AND AGREES TO INDEMNIFY, HOLD HARMLESS, AND DEFEND BRUSH PRAIRIE BAPTIST CHURCH FROM ANY AND ALL CLAIMS AND DAMAGES RELATING THERETO. THE REQUESTOR AGREES THAT ANY DISPUTES ARISING UNDER THIS AGREEMENT WILL BE RESOLVED VIA A MUTUALLY ACCEPTABLE ALTERNATIVE DISPUTE RESOLUTION PROCESS OR THE AMERICAN ARBITRATION ASSOCIATION.
