Name of product you want to receive in exchange, size needed, and specific SKU number of the piece of clothing. SKU numbers are found on the products page after clicking the needed size. (ex: U100-SML-01)

REFUND TERMS AND CONDITIONS:

Please submit this form via email to leadthewavellc@gmail.com , fill in the subject line with your order number and the word "exchange". Once we have reviewed the exchange form, we will email you a shipping label to print out and attach to the return package. When we have received and confirmed the item(s) condition LTW will then process the exchange. Exchanges must be equal or lesser value of the original purchase price. Exchanges will ship within 5-7 business days upon the date of return.
