Thank you for taking the time to fill out this form. This will help us better understand what type of extensions you are interested in before an in person consultation.

Our in person consultation appointment are $35. This will be credited back to you on the day of your booked service.

If, during your consultation, you decide to schedule the installation for hair extensions, there will be a deposit for the appointment as well as payment for the purchase of the hair.

We offer a variety of hair extensions from tape in, clip in, fusion, beaded, micro fusion and hand tied wefts. The quality of hair we use is silky smooth so it blends perfectly and is 100% remy human hair.

PLEASE NOTE: The Level Stylists below have limited availabilties and wait times to schedule an install may vary.

*We do offer move up appointments for clients that already have extensions. Please indicate this in the form below.

We offer various hair extension methods, with a $35 consultation required to determine the best option for your hair.

Initial services start at $1200+ (including human hair).
Maintenance adjustments are recommended every 6-8 weeks, starting at $200+.
Additional charges apply for custom coloring, cutting, and styling of extensions.

There are no refunds on installed, colored, or cut hair extensions. For any issues, please refer to our Satisfaction Policy.
