Case Number
Case ("DR") numbers are generated for incidents where formal law enforcement reports are needed. Involved parties are provided with this number on a "Marsy Card" at the scene when possible. The format for OCSD case numbers is YY-######, where the YY represents the last two digits in the current years, and #'s represent numeric digits. You may been provided with a Call Log number only. If so, leave this item blank and see the next item.
Call Log Number
Call Log numbers are generated for every incident reported to the department. When formal reports are not required, parties often choose to request the informal call log, which is a record of the sequence of events that occurred on an incident. Involved parties are provided with this number on a "Marsy Card" at the scene when possible. The format for OCSD call logs is #######-####, where the #'s represent numeric digits. If you do not have a call log number, please leave this item blank.
Incident Details
Please provide any or all of the following information: Date of the incident, approximate time of the incident, the type of incident, the name(s) of part(y)(ies) involved, the type(s) and license(s) of vehicle(s) involved, names of any of the deput(y)(ies) involved, and the resolution of the incident (citation, arrest, restraining order).

After your report request is reviewed, we will provide our response by email or standard mail. Replies delivered by email are free while mailed responses incur a nominal $0.15/page charge. In person puck-up is not currently offered due to COVID-19 restrictions. Please choose your method of delivery.

If your request is denied, a denial notice will be sent by US Mail. If your request is approved or partially approved, you will be contacted with the number of pages to be sent along with the total owed. Payment by check should be sent to: OCSD Records Division, Subpoena and Admin Unit, 320 N. Flower St., 3rd Floor, Santa Ana, CA 92703. Please reference your case or call number on the check. Your paperwork will be released to be mailed once payment is received.

Your credit card will be charged once your request has been reviewed. You will be charged $0.15 per page for all approved or partially-approved pages of documentation and your documents will be sent by US Mail once the charge is approved.
