About Our Studio Portrait Sessions

(please read in full prior to submitting this form)

How it works.

1. Session confirmation will be sent upon receiving the booking form to confirm the date/time

2. you wil receive an online proof and ordering gallery within 5-7 days after your session. You will have 7 days to view and place your order. If you request to reopen your gallery after the 7 days a fee of $50 may be charged.

3. allow up to 3 weeks to recieve your finished package. Harvest Moon will contact you when your order is ready to pick up.

4. all images chosen for your package will be basic retouched. Any special requests such as head swaps, color changes, wardrobe issues ect will be subject to a editing fee.

5. all sessions will include your choice of 2 backdrops

6. only bring people listed on this form to your session.

7. arrive early or on time, and please be fully ready to start your session at your scheduled time.

8. bring coordinating wardrobe for the backdrop sets. Do not forget shoes and proper socks for the family.

9. OUTDOOR SESSIONS are not included and can not be scheduled using this booking form

10. CORPERATE HEAD SHOTS can be booked as a studio session. Please note that any digital images purchased will be realeased in resolution suitable for usage such as flyers, business cards, web, and small usage. High resolution digital images for corperate usage such as billboards, benchs, large printing will have a $99 additional fee applied per digital image.

COPYRIGHT: As a client of Harvest Moon Photography, I acknowledge and understand that the images taken by Harvest Moon Photography, PHOTOGRAPHER; Jeannine Zeschuk, are protected by Federal Copyright Laws. I understand it is ILLEGAL to copy or reproduce these photographs elsewhere without the PHOTOGRAPHER's permission, and this violates Federal Law and may be subject to civil and criminal penalties. A limited PRINT RELEASE is supplied with the purchase of my session digital files to print your portraits for your own personal use. As a client of Harvest Moon Photography I understand that any digital images purchased will be sized for self printing up to 8x10in and all enlargements are to be produced exclusively through Harvest Moon Photography. The print release does not allow publication of your portraits (i.e. entering in contests), selling or altering your images without the PHOTOGRAPHER's permission .Harvest Moon Photography is not responsible for resolution, colour or print quality if produced at any professional or consumer lab, other than produced by them, Reproduction includes, but is not limited to scanning, downloading, right-clicking, emailing, saving , posting on websites or blogs, photocopying, obtaining screen shots, or printing in any manner. I agree to honor the copyright laws as stated in this agreement.*

PORTRAIT AGREEMENT & MODEL RELEASE: I have read and agree to the following statement: I agree to allow Harvest Moon Photography, to photograph myself and/or my underage children. I hereby grant Harvest Moon Photography, the irrevocable and unrestricted right to use and publish photographs of myself, family, and/or child(ren), or in which myself, family, and/or children will be included for studio display, advertising, and any other purpose they see fit. I hereby release Harvest Moon Photography and her legal representatives and assigns from all claims and liability relating to said photographs.*

Photo Session Payment

Session date will be held upon payment and submission of this form. All payments are non refundable unless in the case of emergency/severe illness on the clients part or the photographers part. We will do our best to reschedule your session if a emergency/illness should arise. We will accept cash, debit and email tranfer on request(An invoice will be sent to your email with payment instuctions).
