Liability Waiver
A: ACTIVITY RISK: I/WE UNDERSTAND THAT presence on Orkestai Farm entails potential for bodily injury, whether from tool-use, weather, physical exertion, unexpected behavior, insect bite, obstacles, infection and other obvious and non-obvious risks.

B: ACCIDENT/MEDICAL INSURANCE: I/WE AGREE THAT: Should any emergency medical treatment be required, I/WE understand that Orkestai Farm Inc. is not responsible for any such incurred expenses.

C: l. LIABILITY RELEASE: I/WE AGREE THAT: In consideration of ORKESTAI FARM allowing myself and/or my child’s participation in these agricultural activities, under the terms set forth herein, I/WE, the parents, for ourselves and/or on behalf of our child(ren) and/or legal ward, heirs, administrators, personal representatives or assigns, do agree to hold harmless, release, and discharge ORKESTAI FARM, its owners, agents, employees, officers, directors, representatives, assigns, members, owners of premises, affiliated organizations, and insurers, and others acting on its behalf (hereinafter, collectively referred to as “Associates”), of and from all claims, demands, causes of action and legal liability, whether the same be known or unknown, anticipated or unanticipated, due to the operations of ORKESTAI FARM, or participation in any of the program’s activities, whether on or off the premises of ORKESTAI FARM, except in the event of ORKESTAI FARM’S gross negligence and willful and wanton misconduct. To the best of our ability we believe that all participants' physical condition is suitable for outdoor activities in the elements.
Lead Participant Must be OVER 18 to sign below after reading this entire document and signs on behalf of all the other listed participants: SIGNERS STATEMENT OF AWARENESSI/WE, THE UNDERSIGNED, HAVE READ AND DO UNDERSTAND THE FOREGOING AGREEMENT, WARNINGS, RELEASE AND ASSUMPTION OF RISK. WE FURTHER ATTEST THAT ALL FACTS RELATING THE PARTICIPANTS’ PHYSICAL CONDITION, EXPERIENCE, AGE ARE TRUE AND ACCURATE.

Edit, use, reproduce, exhibit, and distribute these photographs and/or videos, in whole or in part, in any manner and media now known or hereinafter invented, including, but not limited to, print publications, Orkestai Farm’s website, online social media in support of and/or to promote their programs, services, and mission.

I hereby waive the right to inspect or approve any of the photographs and/or videos. I understand that Orkestai Farm will be the exclusive owner of all such photographs and/or videos and that I will not receive any compensation for the use of the photographs and/or videos :
