UC College Credit Plus Modality Change Form
This form is ONLY for current UC CCP students who:

1. Were admitted to CCP as "in-high school only" and only took UC courses offered at their high school.

2. Now wish to be considered for admission to classes on UC's campus or online.

3. Meet the CCP admissions criteria for online or on-campus admissions. For more details, please refer to our CCP Admissions Requirements

This form must be completed in accordance with our application deadlines. Therefore, the deadlines for submitting a request to change modalities for future semesters are as follows:

Spring Semester deadline = November 1

Summer Semester deadline = April 1
