    Morning Sun Energy Healing! 

    In preparation for your Sound Therapy Session, I request that you answer several basic medical questions to help guide me in your upcoming session(s). All information will be kept strictly confidential and will be on file only for the duration of your treatment. You are welcome to add additional information.

    Please return the completed form via email, or bring a printed copy to leave with me, this will allow us to help create a treatment plan that is catered specifically to you. A completed forms signifies your agreement. Any of these questions can be discussed in person.

    The basics of the first session will include a brief period of talking, where you share information on your health, both past and current, and discuss current issues that are present in your life. Please let me know if there are specific physical or emotional issues you would like to address. (It would be best to call or email to discuss medical issues or specific intentions prior to the session, once you arrive we will begin to gently focus on relaxing and getting comfortable.) 

    Your session should take approximately 45 to 75 minutes. Please turn your cell phone off or leave it in the car. It is advised that you begin becoming well hydrated the day before the session and to drink lots of water after the treatment.

    Please wear comfortable, loose layers of clothing, preferably without metal buttons, belt buckles, or underwire bra, and remove ALL metal accessories and/or jewelry before entering the space (wedding rings need to be removed before Reflexology Sessions).

    Following this, you lie on a massage table fully clothed, and we will begin the work by placing Himalayan Singing Bowls around your head, body, or joints, chakras, and any areas of complaint working off or on your body depending on the session chosen, as the Sound Bowls aid the body’s own innate natural healing processes to align many physical, emotional, and energetic ailments and imbalances.

      I have read, understood, acknowledge and confirm that the information is true and correct and hereby agree to all the above and grant permission to have Sound Healing Treatment(s) performed on me. I understand that Himalyan singing bowls may be placed on and around my body, and voluntarily consent to Sound Healing Treatment(s) from Melissa Schumacher and/or Morning Sun Energy Healing. I intend this consent form to cover the entire course of treatment. I understand that I am free to withdraw my consent at any time.

      By signing below, you acknowledge your understanding that this work is vibrational and energetic. The services that Morning Sun Energy Healing offers are considered alternative and complementary to traditional western medical practices. Morning Sun Energy Healing and/or Melissa Schumacher is not trained as a medical doctor and does not diagnose any medical condition, suggest any medical treatment, or prescribe or recommend any medication. Morning Sun Energy Healing and/or Melissa Schumacher will not interfere with the treatment of a licensed physician or medical professional and these sessions are not a substitute for any medical advice or treatment given by your physician.

      I agree to hold Melissa Schumacher and Morning Sun Energy Healing free and harmless from any claims, suits for damages or complications which may result from such treatment.
