Introduction
Before completing this form, you are advised to read ASLPD214 Probation Review Procedure.
Please return a copy of this form to HR after each review. You must seek the advice of HR as soon as possible if difficulties arise during the probationary period which mean that extending the probationary period and/or an inability to confirm the employee’s position, are possible outcomes. Non-reporting will result in the assumption that the employee’s probation period is progressing satisfactorily.
The line manager should ensure that the employee is given a copy of this document at each stage of their probation and should retain the original to monitor progress against set objectives at follow-up meetings.
Stages of probation review:
Part 1 – Initial Meeting should be held no longer than 2 weeks after employment start date
Part 2 – Second Review should be held no longer than 3 months after employment start date. (This is an informal stage of the process, the Probation Review Form will not be required)
Part 3 – Final Review should be held no longer than 6 months after employment start date
The probationary period may be extended beyond the first 6 months, up to a maximum on a further 3 months.
