I, Full Name* , certify that I am an adult with full legal capacity to express my consent voluntarily as of today,   Date*   and that I have read, and subscribe to the following premises:

I am in a healthy state, which I understand is necessary for the physical activity I will be making while riding a bicycle;*

I am aware of the possibility of having an accident while biking, which could be harmful to me, others or private property. This could be provoked by an unexpected failure of the bike or a part of it, or a collision with another vehicle or being(s).*      

I will not make Cultura Bicicleta Coop responsible for any lawsuits, reclamations, loss, lesions, or harm, I or any other being(s) suffer(s), as a consequence of biking.*     

It will be my responsibility to take care and make up for any resulting damages to the bike(s), and/or other being(s), and/or private property, caused by me or anyone biking under my supervision. *        

I irrevocably authorize Cultura Bicicleta Coop to copy, exhibit, publish or distribute, through all means known or to be known, any and all images, recordings, including artistic compositions or designs, for any lawful purpose, within and outside the jurisdiction of the Commonwealth of Puerto Rico. Furthermore, I cede my right to examine or approve the final product, including texts or transcriptions that may have a resemblance to my person.
