  • This application process has three components:

    1. Submit the online application, completing all sections, uploads, and the supplemental material requested. All information will be kept in strict confidence. It will not be given or sold to anyone, for any purpose beyond evaluation for entrance into the program.

    2. Send personal and pastoral references to your chosen, applicable people; ensure they print, and then scan and email the completed forms back to info@mandate.global, or mail it to the below address.

    • Pastoral: completed by two pastors who are unrelated to the applicant and have known the applicant for at least one year.
    • Personal: completed by someone who is unrelated to the applicant and has known the applicant for at least two years.

    3. We will contact you to arrange a personal, phone or zoom interview.

    All information you provide within this application will be kept confidential. Your information will never be shared, sold, or provided to anyone, and will be used solely for assisting our leadership team in determing if Mandate is the right fit for you.

  • Health Information

    INSURANCE INFORMATION: Mandate does not provide coverage or funds for health care, either for an illness or in case of an accident. We strongly encourage all participants to maintain personal health insurance. Participants who already have insurance through their parents, spouse, or work are advised to be aware of their coverage stipulations and benefits. Some parents' health plans will only provide coverage if the participant receives medical attention in the immediate vicinity in which the parents live. If you are currently covered, please provide the information requested below: Name of Insurance Company, Policy Number, and Claims Department Phone Number. Note if the policy continues to cover you while you are away from home.

    We are asking these personal questions so that if a medical or psychiatric issue arises during your time at Mandate which could adversely affect you or those around you, we will be prepared to support and assist you during this time.

  • Medical Disclosure

    If you are currently on any medications, which are prescribed and being regulated by a doctor we must know for liability reasons. All information you provide will be kept confidential.

I understand that failure to keep taking my medications under my doctor’s care is grounds for my immediate dismissal.   • Additional Funds

    Program tuition includes two meals each day (lunch and dinner) prepared by a cook (Tuesdays through Saturdays except where noted in the schedule). Remaining meals must be provided by the student. NOTE: Due to Newlife Community Church’s weekly Food Ministry program to Pomona, participants have access to food provided for this ministry, which will lower personal food costs. However, it is up to the participant to choose food they enjoy and take it back to their housing.

  • Mandate Guidelines

    Teachability: We ask that every student be teachable, willing to learn, and honest in his/her pursuit of holiness.Vacation: Personal breaks are only permitted for emergencies. Participants are expected to plan ahead to keep the program an uninterrupted season of consecration and impartation.Personal Appearance: Participants are expected to uphold a clean, modest, and non-distracting appearance in their dress for all meetings, sessions, services, and gatherings throughout Mandate. We desire to bring glory to Jesus with the way we physically present ourselves.Health Insurance: Applicants should provide their own health insurance coverage. Mandate is not responsible for covering hospitalization, visits to the doctor, or for providing medications for basic cold/flu, or headaches.Vehicle: Participants are responsible for their timeliness (i.e., they must be punctual for meetings and sessions) whether they have a vehicle or not. Mandate will provide transportation to and from all Mandate related responsibilities, if needed, or you you may use your own vehicle. Personal Expenditures: Participants are required to have sufficient funds to cover all personal and living expenses incurred throughout the program. Due to the nature of the program, simultaneous employment is impossible. Please contact Mandate leadership if you have an extenuating circumstance that needs to be discussed.Foundational Commitments: We recognize that external rules of behavior are not the highest ideal for any Christian community and desire that our motivation for holiness would be love for Jesus and his people, not rules. In this spirit we affirm the following: The Mandate community expects all of its staff members and participants to make a personal commitment to live counter to the prevailing moral laxity of our society by not participating in, advocating, supporting, or condoning sexual activity (heterosexual or homosexual) outside of marriage. Further, we will demonstrate our commitment to Christ and to each other by refraining from the use of tobacco, alcohol, drugs, hookah, or vape. Please reference the Mandate Participant Handbook for our Statement of Faith.

  • Assumption of Risk Waiver COVID-19

    The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited or limited the congregation of groups of people. Newlife Community Church ("the Church") has put in place preventative measures to reduce the spread of COVID-19; however, the Church cannot guarantee that you or your family will not become infected with COVID-19. Further, attending the Church could increase your risk, and your family’s risk, of contracting COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my family and/or I may be exposed to, or infected by COVID-19 by attending the Church or Church-related activities; and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at the Church or Church-related activities may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Church employees, volunteers, and Church-related activity participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my family or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my family may experience or incur in connection with my family's attendance at the Church or participation in Church-related activities ("Claims”). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless the Church, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Church, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Church-related activity.
