Initiation Report Template
- Download a copy of the template.
  - It will come in a .csv file format. You will want to save the file in the same .csv format for uploading.
- Fill out First, Middle, Last, Brother Name and Initiation Date.
  - Format Initation Date as: January 1st, 2021
  - Certificate will be printed exactly as submitted, and is case sensitive, so be sure to have the correct upper and lower case for names and initiation date.
