THE DREAMS COME TRUE MINI-GRANT APPLICATION

The Dreams Come True Mini-Grant is an initiative of the Detroit Wayne Integrated Health Network (DWIHN) Constituents’ Voice Advisory Committee. The Constituents’ Voice (CV) is an advisory group to Detroit Wayne Integrated Health Network. The CV advises DWIHN on its policies, plans, and practices. The grant supports Members in the DWIHN provider network in achieving their personal goals in advancing community inclusion. All Members applying for the grant must receive services within the DWIHN provider network and reside in Wayne County, Michigan, at the time of application submission and throughout the DWIHN application review process. A parent or guardian may apply for a child or youth aged 5-17. Members must provide the necessary identification and documents required by the DWIHN Finance Department if they are selected to receive a grant award. Constituents' Voice members and DWIHN staff are not eligible to apply for The Dreams Come True Mini-Grant.

The Mini-Grant Committee will award up to $500 to each selected individual, not to exceed a total of $5,000 for all mini-grants awarded in FY 2026 (unless there is a tie in scoring). A Member may receive only one grant per year, and only one application per Member will be considered. Applicants under consideration for a 2026 mini-grant must not have been a recipient of a DWIHN Dreams Come True Mini-Grant the previous year (2025). Applications will be accepted from 8:00 a.m. on May 1, 2026, through 5:00 p.m. on June 30, 2026. Incomplete applications or applications received after the deadline will not be reviewed.

Winners will be notified and awarded at the upcoming 2026 Dreams Come True Award Luncheon scheduled for Friday, September 18, 2026. Winners are strongly encouraged to attend the luncheon. All Dreams Come True Mini-Grant 2026 winners must claim and receive their monetary award within 4 months of the 2026 Dreams Come True Luncheon, or they forfeit the award.
