This Client Agreement (the “Agreement”) dated below is between the signed below, herein referred to as the "Athlete", and MVP Academy, LLC herein referred to as “MVP”.
MVP has agreed to provide services to the Athlete on the terms and conditions set out in this Agreement, while the Athlete is of the opinion that MVP has the proper and necessary qualifications, experience and abilities to provide services to the Athlete.
Therefore in consideration of the matters described above, the receipt and sufficiency of which consideration is hereby acknowledged, the Athlete and MVP agree as follows:
1. Scope Of Work
MVP is to provide the Athlete with the following services (the “Services”):
Sports Performance Training
The Services will include any other tasks which the Athlete and MVP may agree on.
2. Term of Agreement
This Agreement will begins on the date signed and will remain in full force and effect until either party decides to terminate or suspend the agreement and either parties discretion.
3. Cancellation
If Athlete seeks cancellation of the Agreement, they must provide notice prior to the First of the month to avoid being charged for the month in question. Athlete fees are subject to change once cancellation is made. There will be a cancellation fee of $25 assessed at the time of any cancellation.
4. Suspension
If Athlete seeks suspension of the Agreement, they must provide notice prior to the First of the month to avoid being charged for the month in question. Each suspension is reviewed on a case to case basis and MVP will accept/deny suspension request at their discretion.
5. Basic Clinet Fees and Compensation
Client use and services must be paid in advance. All schedules and fees are subject to change without notice. Please choose one of the options below:
