T-shirt Order Form

Thank you for considering CandisurDesign! Please complete this form in its entirety. Invoices are sent to the email provided within 24-48 hours. Designing will start within 48-72 hours after payment has been received. Most orders will be processed and mailed within 7-10 business days depending on quantity and complexity of the design. IF YOU NEED YOUR ORDER IN LESS THAN 7 DAYS PLEASE NOTIFY ME!
Mock-ups require a design fee or full payment! It will be emailed for approval before printed/pressed.

**Please note that your shipment can be delayed due to COVID-19. Candisurdesign is not responsible for delayed shipments and no refunds will be provided.

**Please email candisurdesign@gmail.com if this is a rush order and be prepared to pay a rush fee.
