  • This form is to be completed by the homeowner/renter if one or more Girl Scouts in their home, with the supervision and support of her parents/guardians, wishes to hold a Girl Scout cookie booth at their residence.

    If a Girl Scout troop (two or more girls from separate households) wants to hold a Girl Scout cookie booth at a private residence, the homeowner/renter OR Girl Scout co-leader must complete the Private Residence Authorization Form in place of this form.
