Thank you for selecting VoIP Route as your Service Provider
Local Number Portability (LNP) allows you to keep your current telephone number(s) and move them from your current telephone service provider to VoIP Route. You are required to submit this form as verification that you would like VoIP Route to provide telephone service for your business and all associated telephone number(s).
Your current provider requires this Letter of Authorization as proof that you have explicitly requested and authorized to have your telephone number(s) transferred to VoIP Route. By submitting this form, you authorize VoIP Route to initiate the process of transferring your telephone service & telephone number(s) to VoIP Route.
Upon receipt of this form, we will start the transfer process and work with your existing carrier to transfer your service & number(s). Once this request has been received by VoIP Route, any further changes may incur additional fees, please refer to your Porting Policy Agreement. VoIP Route will notify you via email, when a Firm Order Confirmation (FOC) port date has been scheduled.
IMPORTANT: DO NOT submit any service change orders or cancel service with your current service provider until you receive notice from VoIP Route that your number(s) has been successfully ported and are active with VoIP Route. Doing so will cause the port request to be rejected and possibly lose your phone number(s). Once your number(s) has been ported to VoIP Route, you should call your previous carrier to confirm that service has been cancelled.
