The undersigned (“Applicant”) hereby authorizes and consents to a hard pull of credit by Lender (“Lender”) in connection with its loan application, to obtain Applicant’s credit report(s), credit score, and other information necessary for Lender and its agents, contractors, employees, subsidiaries, affiliates, partners, successors or assigns (collectively, “Affiliates”) to assess the creditworthiness of Applicant and whether to approve Applicant’s loan application.
This Authorization to Obtain Credit Report (this “Authorization”) constitutes “written instructions” to Lender and its Affiliates, third party data sources and suppliers, and all other credit reporting agencies under the Fair Credit Reporting Act, as amended, to obtain, access and review Applicant’s credit files from each national credit reporting agency and to exchange information about the Applicant with each such national credit reporting agency in order to verify Applicant’s identity and to administer, review or collect upon the loan. Upon Applicant’s request, Lender shall disclose the name and address of the credit bureau from which Lender obtained a report about Applicant.
Applicant acknowledges and agrees that in connection with this Authorization, its loan application, and, if approved, the servicing of the loan, Applicant and its Affiliates may provide Applicant’s personally identifiable information and other information obtained from the hard pull of credit and Lender’s credit experience information to third parties, including, without limitation, identification verification companies, consumer reporting agencies, credit bureaus, payment validation companies, law enforcement agencies, lenders and prospective lenders to assess Applicant and Applicant’s loan application, to originate or service the loan and/or to offer other products or services that such third parties provide. Applicant may cancel this authorization at any time by sending a letter to Lender to process the request, which letter include Applicant’s full name, address and identification number, if any. Applicant hereby waives all claims against Lender and its Affiliates and third-party data sources and providers for such disclosures and the acts or omissions of these third parties with regard to the use or disclosure of such information.
Applicant hereby certifies that it has read and agreed to all terms, conditions, authorizations and disclosures provided with this loan application and that all information provided is true and correct.
Notice For Ohio Residents: The Ohio laws against discrimination require that all creditors make credit equally available to all credit worthy customers, and that credit reporting agencies maintain separate credit histories on each individual upon request. The Ohio Civil Rights Commission administers compliance with the law.
Notice For Married Wisconsin Residents: No provision of a marital property agreement (pre-marital agreement), a unilateral statement under Section 766.59 of the Wisconsin Statutes, or a court decree under Section 766.70 of the Wisconsin Statutes adversely affects the interest of the creditor unless, prior to the time the credit is granted, the creditor is furnished a copy of the marital property agreement, statement, or decree or has actual knowledge of the adverse provision when the obligation to the creditor is incurred. If the loan for which Applicant is applying is granted, Applicant’s spouse will also receive notification that credit has been extended to Applicant.
